Cannot fully configure an Access Point Name to use the Internet

I cannot enter all parameters to configure the Access Point Name to use the mobile data :

APN : orange
Proxy: Undefined
Port: Undefined
Username: orange
Password: orange
MCC: 208
MNC : 01
Authentication type : PAP
APN type (access point name): default
APN protocol: IPv4
APN roaming protocol: IPv4

The current form contains only 4 fields (name, APN, username and password).
