db: Add chain_index session column
We don't currently use the column, but let's add it as we will require it later and we don't need to add updation path now as we don't yet ship it anywhere.
Due to an influx of spam, we have had to impose restrictions on new accounts. Please see this wiki page for instructions on how to get full permissions. Sorry for the inconvenience.
